Coaching Applications

North Coquitlam United SC, our governing District and BCSA requires all Coaches, Assistant Coaches and Managers over the age of 18 to complete a criminal record check every 36 months. Forms for returning coaches are due each season no later than March 1st. For coaches and managers U12 - U18, we will also require a small photo (1x1 hard copy) for your BCSA ID Card.

New Coaches to North Coquitlam United SC at the non-competitive level (U5 - U12) will have until August 1st to submit your criminal record check. There will be no exceptions.

You will not be assigned or offered a field or gym for practices, games or try outs until the completed forms are to NCUSC and have been confirmed by the Risk Manager. Please include the team name, gender, age and level you are submitting forms for. DO NOT give your forms to your VP's they will not be accepted. If you have questions or concerns please contact the Risk Manager at riskmanager@ncusc.ca.

Coaches and Managers must resgister online through our Member Login for the upcoming season prior to May 30th. You must register for each age group you are coaching/managing. Please update any changes to your contact information if needed. If you have forgotten your username and password it can be retrieved by going to the Member Login and clicking on Forgot Password.

Forms can be scanned and emailed to riskmanager@ncusc.ca. Forms may also be mailed to the Risk Manager or dropped off at:

IMPORTANT: The RCMP now require another step in the criminal record check process. You now need to submit this personalized letter along with your completed criminal record check before the RCMP will start the process and waive the fee.
